Cardiac Rehab
What is Cardiac Rehab?
Cardiac rehabilitation is a program designed to help you improve your health and decrease your cardiac risk factors.
Cardiac rehabilitation is for patients that are getting better after heart problems or heart surgery. It incorporates a personalized exercise program based on your health and your doctor's recommendations.
Cardiac rehabilitation includes different phases: inpatient, outpatient and maintenance. The inpatient phase begins while you are in the hospital and ends when you are discharged home.
Adair County Health System is pleased to offer both outpatient and maintenance programs.
The outpatient program is a rehabilitation that takes place after you are discharged from the hospital. Most insurances cover outpatient cardiac rehabilitation for heart surgeries, heart attacks, stents, angina and other heart problems.
Once you have graduated from our outpatient program, we offer a maintenance program (C.A.R.E.+) designed to maintain your increased cardiac health and fitness.

Cardiac Rehab Benefits.

What happens in an outpatient rehab program?
        •    Exercise 2-3 times per week using a treadmill, bike or rowing machine.
        •    Start slowly and gradually build as you become stronger.
        •    Begin strength or resistance training when okay with your doctor.
        •    A specially trained nurse will monitor your heart, blood pressure and other symptoms.
        •    Receive education on numerous heart healthy topics, including heart disease, risk factors, diet,
               exercise, stress management, smoking and medications.

After you have completed the outpatient program, you may feel better than ever. Make these lifestyle changes a part of your everyday life and continue with our Maintenance Program, C.A.R.E.+

Cardiac Rehab and C.A.R.E.+ appointments can be scheduled 8:00am to 3:00 pm on Monday, Wednesday and Friday. Other days and  times may be available by special request.

Pulmonary Rehab

 

Pulmonary Rehabilitation

If you have a chronic lung disease, shortness of breath (SOB) can be caused by daily activities such as climbing the stairs, making a bed, walking or showering.  This can affect your ability to perform activities of your daily life.  Decreased activity can result in increased SOB.

          The benefits of Pulmonary Rehab can include:

Ψ   Improved ability to perform daily activities

Ψ   Ability to make and adhere to healthier lifestyle choices

 

Who is eligible for Pulmonary Rehab?

In order for Medicare or Private insurance to consider payment of pulmonary rehab, there is certain criteria that must be met.

Medicare:  Covered for patients who have had any of the following:  COPD, Asthma, or Restrictive Pulmonary disease

Private Insurance:  Criteria varies between the companies.  This will be evaluated by the staff prior to the patient incurring expenses.

Out of Pocket Expenses:  Co-pays may be applicable for patients if the patient only has Medicare as a primary insurance.  If there are to be any out of pocket expenses, staff will assist in evaluating this. After the patient has “graduated” to a maintenance program such as the CARE + program, all cost of the program will be paid by the patient.             

 

Pulmonary Rehabilitation includes:

Ψ   Supervised exercise by a healthcare professional

Ψ   Functional and Activities of Daily living

Ψ   Conservation of  Energy

Ψ   Breathing retraining

Ψ   Management of Stress

Ψ   Medication Education

 

In Summary:  Pulmonary Rehabilitation is more than just a supervised exercise and lifestyle modification program.   It is a multi-discipline approach to disease management.  It is an outpatient service that may be covered by your insurance. 

Referral from a physician is required.

Appointments for two to three times a week would be scheduled.

C.A.R.E. +

What is C.A.R.E.+
Cardiac and Rehabilitation Exercise Plus is a fitness program to improve your health and decrease you risk factors for developing cardiovascular disease.

    Do you have:

    
•    High blood pressure
    
•    High Cholesterol
    
•    Diabetes or Pre Diabetes
    
•    Family history of heart problems
    
•    A desire to improve your general health
 
If any of these apply to you, then you could benefit from participating in Adair County Health System's C.A.R.E.+ Program

Why should I participate in C.A.R.E.+
  •    Cardiac and Rehabilitation Exercise Plus is an exercise program that focuses on increasing your health and fitness while decreasing your risk for cardiac disease, diabetes, high blood pressure and stroke.
  •    You will participate in medically supervised exercise and strength training sessions 1-3 times per week.
  •    A nurse is always present while you are exercising. We can check your blood pressure, heart rate, answer medical questions, communicate with your physician, and much more.

Who can Participate in C.A.R.E.+?
ANYONE interested in improving his or her general health can participate. Those attending C.A.R.E.+ must be stable on current medical treatment and be able to exercise independantly. All participants must receive medical clearance from their physician.  Exercise times are set for the convenience of all parties involved.

Cost for the program    $5.00 for a single session        $55.00 for 12 sessions (savings of $5.00)
